ArchWell Health is seeking a Social Work Coordinator in Phoenix to support members’ social, emotional, and practical needs. The role partners with LMSW/LCSW teams to connect seniors with resources, assist with forms, and address Social Determinants of Health.

The candidate should be empathetic, organized, and community-oriented. Daily duties include outreach, resource navigation, care coordination support, and collaboration with care teams.

Qualifications

• Must have knowledge of community resources and Social Determinants of Health (SDOH).
• Excellent verbal and written communication with diverse populations.
• Empathetic, patient-centered with ability to build trust with seniors and families.
• Experience in care coordination and interdisciplinary team collaboration preferred.

Responsibilities

• Conduct outreach calls to assess needs related to SDOH and support services.
• Connect members with community resources including housing, transportation, and food assistance.
• Assist Social Workers with discharge planning and care coordination activities.
• Help members navigate benefits and community programs.
