Qualifications

• HS diploma or equivalent required; healthcare exposure preferred.
• 2 years of referral management or care coordination experience.
• Medical terminology knowledge beneficial.
• Experience with EMR systems (ideally eClinicalWorks) preferred.

Responsibilities

• Partner with multi-disciplinary care team to understand each member's care needs.
• Coordinate referrals to external providers and services.
• Document member referrals to maintain accurate records.
• Build and nurture relationships with members and families to facilitate care coordination.
• Monitor care plans and follow up on goals as outlined.
• Adhere to HIPAA and protect PHI.
