• Build and manage schedules for clients and Care Professionals, focusing on quality matches and continuity of care.
• Answer incoming calls with warmth and professionalism.
• Monitor and respond to scheduling changes quickly, including evenings and weekends as needed.
• Coordinate with clients, families, and Care Professionals to resolve issues before they become problems.
• Maintain accurate records in scheduling and care management systems.
• Identify opportunities to expand and improve care for existing clients.
