ArchWell Health is seeking a Referral Coordinator to support our members as they navigate the healthcare system, guiding families to the right specialists and ensuring timely care. The role requires strong knowledge of the healthcare ecosystem, attention to detail, and a passion for seniors' care.

You will partner with a multidisciplinary team, coordinate referrals, document processes, and promote clear communication to achieve positive member outcomes.

Qualifications

• HS diploma required; healthcare coursework preferred.
• 2 years of referral management or care coordination experience.
• EMR experience, preferably eClinicalWorks.
• Fluency in Spanish or languages spoken in the service communities.

Responsabilités

• Coordinate referrals to external providers and services to meet care needs.
• Document member referrals to maintain accurate and current records.
• Collaborate with the care team to identify gaps and barriers to treatment.
• Maintain HIPAA compliance and protect PHI.
• Serve as navigator for members and families, facilitating questions and concerns.
