Job summary

ArchWell Health is seeking a Referral Coordinator to support senior members navigating the healthcare system. You will guide families to the right specialists, coordinate referrals, and ensure timely, appropriate care inside or outside our centers.

The role requires deep healthcare ecosystem knowledge, attention to detail, and strong communication across diverse populations, with emphasis on HIPAA compliance and care plan follow-through.

Qualifications

• High school diploma or equivalent; healthcare coursework preferred.
• Minimum 2 years in referral management or care coordination.
• Medical Assistant or medical admin experience is helpful.
• Experience with EMR systems such as eClinicalWorks or similar.

Responsibilities

• Partner with care team to understand member care needs and barriers.
• Coordinate referrals to external providers and services.
• Document referrals to keep records accurate and current.
• Build relationships with members and families to facilitate coordination
• Monitor care plans and follow up on goals per the plan.
• Adhere to HIPAA and protect PHI at all times.
• Promote clear communication among care team and clinicians.
• Assist members with problems accessing care, transportation, or meds.
• Serve as system navigator for members and families.
