Qualifications

• Deliver preventive and comprehensive primary care to adults 65+ (Medicare Advantage).
• Develop individualized care plans and manage chronic conditions.
• Lead the integrated care team and support quality initiatives.
• Participate in onboarding covering value-based care and EMR workflows.
