gobravvo is seeking a Primary Care Physician (Internal Medicine or Family Medicine) to deliver comprehensive, preventive, and patient-centered care for adults 65+. The role emphasizes value-based care, extended appointment times, and teamwork with NPs, PAs, and other staff in a mission-driven setting.

The practice focuses on chronic disease management, HEDIS quality measures, and high-quality patient outcomes, supported by onboarding and ongoing professional development.

Formación

• MD or DO degree with Board Certification or Board Eligibility in Internal Medicine or Family Medicine.
• Geriatric experience or senior population care is a plus.
• HEDIS or value-based care experience is preferred but training is provided.

Responsabilidades

• Provide comprehensive primary care to adults 65+ focusing on prevention and chronic disease management.
• Conduct thorough assessments and develop individualized care plans.
