Job summary

gobravvo is seeking a Primary Care Physician (PCP) to deliver value-based, patient-centered care to Medicare populations within a collaborative clinic setting. You will work with a multidisciplinary team to improve outcomes while focusing on preventive medicine and reducing hospital admissions.

The role emphasizes quality metrics, physician partnership tracks, and comprehensive benefits within a supportive environment that includes teleconsultation and onsite labs.

Qualifications

• MD or DO required.
• Board certified or board eligible.
• Active, unrestricted state medical license.
• Fellowship training in Geriatrics preferred but not required.
• Not accepting residents for this role.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Manage a daily patient panel of 13-16, balancing regular and new patient visits.
• Collaborate with care teams, including medical assistants, scribes, and social workers.
• Utilize onsite labs and teleconsultation for efficient, comprehensive care.
• Document encounters using Canopy and Greenway EMR systems.
• Engage patients in community wellness activities and facilitate access to care through transportation services.
• Participate in clinic meetings, quality improvement initiatives, and professional development opportunities.
