Job summary

gobravvo is seeking a Primary Care Physician with a background in Internal Medicine or Family Medicine to join a progressive value-based care organization focused on adults 65+ and Medicare Advantage patients. Qualifications

• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Geriatric experience preferred but not required
• HEDIS experience is a plus
• Value-Based Care experience is preferred with training provided

Responsibilities

• Provide primary care services for adults 65+, focusing on preventive care and chronic disease management
• Develop individualized care plans and coordinate with the integrated care team
• Document patient encounters following EMR best practices
• Participate in value-based care initiatives and quality improvement programs
• Engage in ongoing training and professional development
