Job summary

Heartline Staffing is seeking a Primary Care Physician to provide preventive primary care for adults 65+ with a focus on Medicare Advantage, chronic disease management, and preventive care. You will see 12–15 patients per day with 20–40 minute visits in an integrated care team setting.

Training is provided for Value-Based Care. Benefits begin Day 1, with compensation including base salary and bonuses; relocation assistance is available for moves >50 miles.

Qualifications

• Board Certified or Board Eligible in Internal Medicine or Family Medicine REQUIRED.
• Comfortable practicing within a value-based care model.
• Strong communication and team-oriented mindset.

Responsibilities

• Provide primary care for adults 65+.
• Focus on chronic disease management and preventive care.
• Practice within an integrated care model.
• Work collaboratively with NP/PAs, Medical Assistants, Social Workers, lab and pharmacy support, and referral coordination.
