Job summary

WelbeHealth PACE is seeking a Primary Care Provider (NP/PA) to serve seniors in home and community settings. The role emphasizes value-based care, close collaboration with the IDT, and long-term patient relationships.

The provider will perform assessments, plan primary care, educate participants and families, and coordinate with WelbeHealth physicians to optimize care across settings.

Qualifications

• Must be a licensed Nurse Practitioner or Physician Assistant with prescriptive authority.
• Demonstrated knowledge of geriatrics, chronic care, and best practices.

Responsibilities

• Provide continuous medical coverage and conduct comprehensive in-person assessments for participants.
• Collaborate with physicians and IDT to develop care plans and ensure effective care coordination and discharge planning.
• Perform regular visits to participants in skilled nursing facilities and homes as needed.
• Review outcomes and utilization data to drive policy improvements and best practices.
