Job summary

WelbeHealth seeks a Primary Care Provider - Nurse Practitioner or Physician Assistant in San Jose, California. This full-time role collaborates with WelbeHealth physicians and the IDT to perform assessments, plan primary care, and deliver ongoing medical care to participants.

Responsibilities include in-person comprehensive assessments, care plan development with the care team, discharge planning, regular visits to participants in nursing home settings, and ensuring care coordination.

Qualifications

• Must have NP with prescriptive authority or PA license and licensure in California.
• Minimum two years of experience working with the senior population.
• Current knowledge of chronic care/geriatric issues and best practices.

Responsibilities

• Perform in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments, and continuous medical coverage.
• Collaborate with physicians and IDT to develop care plans for each participant.
• Collaborate with physicians and IDT in discharge planning from acute or long-term settings.
• Maintain care of participants in nursing homes with regular visits as needed.
• Oversee care coordination with WelbeHealth physicians, community-based PACE physicians, and IDT.
• Review outcomes and utilization data to establish best practices and policy improvements.
• Participate in rotating night and weekend call.
