Job summary

WelbeHealth is seeking a Primary Care Physician to deliver life-changing, community-based care to our vulnerable seniors within a value-based PACE program. You will build long-term relationships with patients, coordinate discharges, and lead comprehensive assessments with an interdisciplinary team.

You will work regular hours 8am-5pm, Monday–Friday, with home visits and rotating on-call duties. Relocation assistance, CME reimbursement, and a strong benefits package are included as part of

Qualifications

• Must have completed an ACGME-accredited residency program.
• Master's or doctorate degree.
• Board certification preferred.
• Active state medical license without restrictions.
• State CDS certification and DEA registration.

Responsibilities

• Perform comprehensive in-person assessments with the IDT to develop individualized care plans.
• Coordinate discharge planning for acute and long-term placements.
• Focus on comprehensive, non-hospital rounds, seeing an average of 8 patients per day.
