Job summary

WelbeHealth PACE in the United States seeks a Primary Care Provider to join our interdisciplinary IDT. You will provide continuous medical coverage, perform comprehensive in-person assessments, and help develop participant care plans to support aging in place.

The role requires an accredited nursing program graduate, NP or PA with prescriptive authority, an active RN license, and at least two years of geriatric care experience.

Qualifications

• Graduation from an accredited school of nursing.
• Certified Nurse Practitioner or Physician Assistant with prescriptive authority.
• Active RN license and geriatric care experience.

Responsibilities

• Provide continuous medical coverage and perform comprehensive in-person assessments.
• Collaborate with Physicians and the IDT to develop participant care plans.
• Oversee care coordination and discharge planning from acute or long-term settings.
• Review outcomes and contribute to best practices and policy improvements.
