Job summary

ChenMed is seeking a Primary Care Physician (PCP) to provide high-quality care for older adults within our outcomes-focused, value-based model. You will lead a care team, coordinate with hospitalists, and drive health outcomes for a panel of patients while advancing policies and quality initiatives.

You will work within a physician-led organization that emphasizes accountability, coaching for health, and simplifying care into actionable steps for patients and care teams.

Qualifications

• Fluency in English, verbal and written.
• Strong leadership and accountability for outcomes.
• Ability to work independently and with care teams.
• Comfort with travel locally and nationally for training and recruiting.
• Demonstrated commitment to evidence-based medicine and quality care.

Responsibilities

• The PCP independently provides care for patients with acute and chronic illnesses in older adults.
• Assumes full accountability for patient care and outcomes and seeks consultations as needed.
• Engages with hospitalists when patients are in hospital and coordinates with the care team.
• Manages assessment, diagnosis, treatment, education, health promotion and care coordination.
• Leads the care team: medical assistant, care facilitator and care coordinator.
• Coordinates care for in-hospital, SNF, LTC or homebound patients with transitional-care resources.
• Supports center management and participates in recruiting patients and providers.
