SCAN seeks an experienced Advanced Practice Clinician (APC) to join our Riverside, CA healthcare team. You will deliver high-quality patient care, collaborate with physicians and staff, and ensure adherence to latest medical standards. This role targets high-risk patients in partnership with the Regional Medical Director.

Expect 75% travel, remote/field work, and ongoing professional development. Strong clinical skills and EHR proficiency are essential for success in this role.

Qualifications

• Must hold CA RN license and complete APC program.
• 75% travel to Riverside area.
• Strong clinical knowledge and ability to diagnose and treat chronic diseases.

Responsibilities

• Conduct comprehensive patient assessments and develop treatment plans.
• Collaborate with physicians, nurses and healthcare professionals for coordinated care.
• Participate in quality improvement initiatives and research to enhance care.
• Maintain accurate patient records and ensure regulatory compliance.
• Stay current with medical knowledge through ongoing education.
