Job summary

SCAN Health Plan seeks an experienced Advanced Practice Clinician (APC) to join its Riverside, CA team. The APC will perform comprehensive patient assessments, develop treatment plans, prescribe therapies and coordinate with physicians, nurses and other staff to ensure high-quality care for high-risk patients.

The role requires 3–5 years of related experience, CA RN license, and completion of an APC program; travel 75% in the Riverside area is expected.

Qualifications

• BSN preferred; RN in CA required; APC program completed; APC certification preferred.

Responsibilities

• Provide high-quality patient care and develop treatment plans.
