Job summary

Gary and Mary West PACE in California seeks a Care Navigator to support the IDT by coordinating the Service Delivery Request process and documenting meeting discussions. You will assist participants by connecting them to the PACE system of care and proactively resolving disenrollment concerns.

The role requires two years of healthcare experience with frail elderly, strong communication, and the ability to work with internal and external stakeholders.

Qualifications

• High school diploma or GED required.
• Bachelor's degree in health or human services preferred.
• Active driver license and clean driving record.
• Must pass pre-employment drug/alcohol screening and criminal background check.

Responsibilities

• Participate in IDT meetings and provide support to the IDT.
• Scribes IDT meeting agenda and assists with coordinating meeting.
• Coordinates transportation schedule in IDT and tracks cancellations.
• Manages Service Delivery Requests including initiation in EMR, documentation, NOA letters for denials, and service dates.
• Leads Retention Committee and acts as liaison between disciplines, services and participants.
