Job summary

Gary and Mary West PACE in California is seeking a Nurse Practitioner to provide primary care and acute illness evaluation for PACE participants in a multidisciplinary team.

You will conduct assessments, coordinate care plans with the IDT, participate in night and weekend calls, and help reduce hospitalizations while maintaining patient privacy and licensure.

Qualifications

• Graduate of an accredited school of nursing.
• RN state license required.
• Nurse Practitioner with prescriptive authority and DEA.
• Current BLS and CPR certification.
• Valid driver's license and proof of auto insurance.

Responsibilities

• Provide primary care and acute illness evaluation and treatment in the center and in participants' homes.
• In collaboration with a physician, perform in-person comprehensive assessments and develop a care plan with the IDT.
• Collaborate with physicians and IDT in discharge planning from acute or long-term care settings.
• Refer participants to medical specialists as indicated.
• Manage care of participants in the nursing home with regular visits as needed.
• Be an active member of the IDT communicating participant changes and coordinating 24-hour care delivery.
• Participate in rotating night and weekend calls.
• Review outcomes and utilization data to improve care and reduce unnecessary hospitalizations.
