Gary and Mary West PACE in North County San Diego seeks a Clinical Supervisor to lead Nurse Practitioners and Physician Assistants within the PACE program. You will provide clinical leadership, coaching, and performance management while maintaining a reduced participant panel.

You will collaborate with the Medical Director and the IDT, promote evidence-based care, ensure regulatory compliance, and support workflow improvements to deliver high-quality, participant-centered services to frail

Qualifikationen

• Minimum five years of clinical NP/PA experience in adult chronic disease management.
• Two years of leadership or supervisory experience.
• Experience with frail elderly population preferred.
• Strong collaboration with IDT and leadership.
• Knowledge of chronic care and palliative care would be a bonus.

Aufgaben

• Direct supervision of Providers (Nurse Practitioners and Physician Assistants).
• Coordinate with Medical Director and Providers to ensure consistent clinical practice.
• Recruitment, onboarding, and performance evaluations for Providers.
• Support quality improvement and regulatory compliance efforts.
• Develop and update clinical protocols and care plans.
