Job summary

West PACE in San Marcos, CA is seeking a Care Navigator to support the IDT, coordinate Service Delivery Requests in EMR, and help participants access needed services.

You will work with a multidisciplinary team, promote continuity of care, and reach out to participants to resolve concerns, aiming to prevent disenrollment.

Qualifications

• High school diploma or GED required.
• Bachelor's Degree in health or human services preferred.
• Active driver license and clean driving record.
• Proof of auto insurance.
• Must pass pre-employment drug screen and background check.

Responsibilities

• Participate in IDT meetings and support the IDT.
• Coordinate transportation schedule in IDT and track cancellations.
• Lead Retention Committee and liaise between disciplines, services, and participants to ensure continuity of care.
• Manage Service Delivery Requests in EMR and document, NOA letters for denials, and service dates.
• Communicate with participants, families, and care team to resolve concerns.
