Job summary

Gary and Mary West PACE in San Marcos, CA seeks a Physician Assistant to provide primary and acute care to PACE participants in an interdisciplinary team. You will perform comprehensive assessments, coordinate with IDT, and manage in-center and home-based care under medical direction.

Qualifications

• Graduate of an accredited Physician Assistant program.
• Physician Assistant State License.
• Valid DEA.
• Certification as a Physician Assistant (PA)/Provider with prescriptive authority.
• Current Basic Life Support (BLS) and CPR training.

Responsibilities

• Provide primary care and acute illness evaluation and treatment for participants.
• Perform in-person comprehensive assessments and coordinate with the IDT to develop a care plan.
• Collaborate with physicians and IDT on discharge planning and referrals.
• Manage care of participants in nursing homes and home visits as needed.
• Participate in rotating night and weekend calls.
• Review outcomes and utilization data to improve care and reduce hospitalizations.
