You will be the primary contact, conduct structured interviews, perform home visits, assess social needs, and support transitions of care across settings. This role emphasizes cultural competence, bilingual English/Spanish abilities, and collaboration with primary care, pharmacists, and community resources.

Qualifications

• 3+ years in ambulatory, primary care, or senior-care with direct patient care.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Willingness to conduct home visits and community outreach.
• Experience in patient education, care coordination, and social support for high-risk or geriatric populations.

Responsibilities

• Conduct structured patient interviews and document findings for providers.
• Perform home visits to assess living conditions and safety.
• Identify barriers to care and connect patients with community resources.
• Deliver education using approved materials for chronic disease management.
• Coordinate care between patients, primary care, specialists, pharmacies, and home health.
• Support care transitions and follow-up appointments, closing care gaps.
• Engage with communities to reinforce health goals.
• Apply 5Ms framework to understand patient needs.
• Participate in High‑Risk Rounds.
