Humana Inc's Conviva Senior Primary Care is seeking a Care Coach to coordinate care for high-risk Medicare Advantage members in the Oakland area. You will act as the primary contact, perform home visits, support social needs, and reinforce care plans across providers.

This role combines clinic-based and field work, requires 3+ years in ambulatory care, and may require bilingual English/Spanish, with a focus on culturally competent, patient-centered care and safe transitions.

Qualifications

• Healthcare professional with 3+ years of ambulatory or primary care experience.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfortable with home visits and community-based outreach.
• Experience in patient education, care coordination, and social support for high-risk or geriatric populations.

Responsibilities

• Coordinate care across health and social service systems, advocating for patients.
• Perform home visits to assess living conditions and safety factors.
• Identify barriers to care and connect patients with community resources.
• Deliver chronic disease education and reinforce treatment plans.
• Serve as liaison among patients, primary care, specialists, pharmacies and home health.
• Support care transitions and follow-up across care settings.
