Job summary

Humana, Inc. is seeking a Care Coach to provide proactive, patient-centered care coordination for high-risk seniors within Conviva Senior Primary Care.

You will serve as the primary contact for patients, conducting screenings, coordinating care across providers, and supporting transitions of care. This role involves home visits, community outreach, and collaboration with primary care teams to reinforce care plans.

Qualifications

• 3+ years of ambulatory or primary care experience with direct patient care.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfortable conducting home visits and community-based outreach.
• Experience in patient education, care coordination, and social support of high-risk or geriatric populations.

Responsibilities

• Coordinate care across health and social service systems, serving as patient advocate and clinical support.
• Conduct structured patient interviews and document health-related information.
• Perform home visits to assess living conditions and social factors.
• Support care transitions and follow-up appointments across care settings.
• Engage with community resources to address social needs and barriers to care.
