Humana Inc. seeks a Care Coach to provide proactive, patient-centered care coordination for high-risk seniors. You will be the primary contact, conduct home visits, reinforce care plans, and support transitions of care across settings.

Responsibilities include chronic disease education, coordinating with primary care, specialists, and pharmacies, and engaging with community-based resources to reduce gaps in care.

Formación

• 3+ years of Ambulatory, Primary Care, or Senior-Care experience with direct patient care.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfortable with home visits and community-based outreach.
• Experience in patient education, care coordination, and social support for high-risk or geriatric populations.

Responsabilidades

• Conduct structured patient interviews and collect health-related information.
• Perform home visits to observe living conditions and identify barriers to engagement.
• Identify social needs and connect patients with community resources.
