The role requires a BA/BS and CP software knowledge, with IDOA certification upon hire and travel within assigned communities.

Qualifications

• Bachelor's degree in arts/sciences or related field required at hire.
• Strong communication, problem solving and critical thinking skills.

Responsibilities

• Engage and assess the complex needs of elderly, frail and clients in community settings.
• Develop care plans and connect clients to supports and resources.
• Conduct timely assessments and provide case management services.
• Maintain care plans, documentation, and client confidentiality.
• Travel independently within assigned communities to perform duties.
• Collaborate with internal stakeholders and participate in trainings.
