Job summary

Synergy HomeCare of Northeast Chicago is seeking an experienced Care Coordinator / Case Manager to join our team. The role focuses on client intake, coordinating services, and building strong referral relationships across Cook County.

Ideal candidates have IDOA CCP/CCU/MCO experience or work in AAA or Medicaid settings and are looking to move into a provider-side leadership role. A nursing license or social work degree is preferred.

Qualifications

• Experience in IDOA CCP, CCU, AAA, Medicaid, or care coordination strongly preferred.
• Background in case management, social work, or home care coordination.
• RN, LPN, or bachelor's degree in social work, Human Services, Public Health, or related field preferred.
• Knowledge of DON assessments, service authorization, and care planning.

Responsibilities

• Manage client intake, onboarding, and service coordination.
• Collaborate with IDOA case managers, CCUs, and MCO care coordinators.
• Ensure compliance with Illinois Department on Aging (IDOA) CCP requirements.
• Communicate with clients, families, and healthcare providers.
• Build and maintain referral relationships with community partners.
