The Care Coordinator conducts timely assessments for elderly and frail clients in community settings, develops and maintains comprehensive care plans, provides ongoing case management and referrals, ensures accurate documentation and certifications, collaborates with internal teams, travels within assigned communities, and engages in continuous learning to support high-quality client care.
