Job summary

ArchWell Health is seeking a Social Work Coordinator to support seniors by connecting them with community resources, assisting with social service needs, and reducing barriers to health and well-being.

Working with LMSW/LCSW, providers, and care teams, you will help navigate resources, benefits, and forms, while addressing SDOH. The ideal candidate is empathetic, organized, and focused on independence for older adults.

Qualifications

• Knowledge of community resources and Social Determinants of Health.
• Strong interpersonal communication with active listening.
• Empathetic, non-judgmental, member-focused.
• Ability to build trust with seniors, caregivers, and partners.
• Organized with strong follow-up and attention to detail.
• Able to navigate multiple priorities while delivering good customer service.
• Experience collaborating in interdisciplinary teams.
• Problem-solving to address member needs.

Responsibilities

• Outreach calls to assess needs related to SDOH.
• Connect members with community resources and services (food, housing, transportation, etc.).
• Assist Social Workers with care coordination, discharge planning, and higher care placement.
• Help members navigate benefits and community programs.
• Serve as a resource guide for members and families seeking assistance.
• Facilitate referrals to organizations and providers.
• Assist with completing forms and documentation for various supports.
• Support services that promote independence, safety, and well-being.
• Collaborate with care teams and community partners on member needs.
• Assist with workshops on SDOH and other topics.
• Maintain accurate documentation in EMR and HIPAA compliance.
