Mercy Retirement & Care Center in Oakland, CA is seeking an MDS Coordinator to manage the resident assessment data collection and nursing documentation. You will review MDS forms, schedule assessments, and ensure timely completion for regulatory compliance.

The role requires an LPN or RN license, a minimum of two years in long-term care, and strong organizational and communication skills. You will collaborate with the care team and participate in admissions and care conferences.

Qualifications

• Bachelor’s degree in related health discipline or equivalent experience.
• Current State Licensing as an LPN or Nurse with at least 2 years in long-term care.
• Excellent organizational and management skills.

Responsabilités

• Manage and coordinate the resident assessment data collection process (MDS).
• Review MDS forms and establish a yearly assessment schedule with Nursing Director.
• Transmit MDS materials to the State electronically and ensure compliance with regulations.
• Consult with residents, families and staff; communicate information clearly.
• Participate in admissions, Medicare, and quarterly care conferences; assist in resident care and evaluations.
