Mercy Retirement & Care Center in Oakland, CA is seeking an MDS Coordinator to manage resident assessment data collection, review MDS forms, and coordinate schedules with the Director of Nursing.

The ideal candidate holds a bachelor's degree in a health field, active LPN or RN license, excellent organizational and communication skills, and CPR/First Aid certification. You will work as part of a collaborative health center team.

Qualifications

• Bachelor's degree in health-related field or equivalent experience.
• Current LPN/RN license with at least two years in long-term care.
• Excellent organizational and management skills.
• Strong English written and spoken communication.
• Must pass background check and pre-employment screenings.

Responsibilities

• Manage and coordinate required resident assessment data collection.
• Assess the status of MDS forms for all residents.
• Establish annual assessment schedule with the Director of Nursing.
• Review results of the last survey to identify improvements.
• Complete nursing section of MDS assessments.
• Transmit MDS materials to the State as required.
• Maintain knowledge of applicable laws regarding RAI/MDS.
• Interact with residents, families and staff; provide clear communications.
• Participate in admission, Medicare and quarterly care conferences.
• Assist in resident care and evaluations as needed.
• Perform other duties as assigned.
