Job summary

The Mariemont Care Center is seeking an experienced MDS Coordinator to join our nursing team. The ideal candidate will oversee MDS assessments, manage the RAI process, and coordinate care planning across departments to ensure high-quality resident care.

Applicants must hold an active RN license in Ohio and have prior MDS experience. Long-term care exposure preferred; strong attention to detail and teamwork are essential to meet regulatory requirements and reimbursement standards.

Qualifications

• Current, active RN license in Ohio.
• Prior MDS experience required.
• Long-term care/skilled nursing experience preferred.
• Knowledge of MDS 3.0, RAI, and care planning.
• Strong attention to detail and organization.
• Ability to work independently and meet deadlines.
• Strong communication and teamwork skills.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time.
• Manage the Resident Assessment Instrument (RAI) process.
• Develop and update individualized care plans.
• Coordinate with nursing, therapy, dietary, social services, and other departments.
• Participate in interdisciplinary care plan meetings.
• Review clinical documentation for accuracy and compliance.
• Monitor MDS schedules and required deadlines.
• Ensure documentation supports resident care and reimbursement.
• Maintain compliance with CMS, Medicare, Medicaid, and state regulations.
