Job summary

Community Medical Centers is seeking an RN MDS Coordinator to manage the comprehensive MDS assessment process for all residents in our facility. You will work closely with an interdisciplinary team and the DON to ensure accuracy and timeliness of data used in care planning and quality reporting.

Requirements include an active California RN license, BLS certification, and at least 2 years of long-term care experience.

Qualifications

• Education: High School Diploma or equivalent; MDS training within 6 months.
• Nursing degree preferred; California RN license required.
• 2 years of long-term care experience; MDS assessments experience preferred.

Responsibilities

• Ensure accurate and timely completion and transmission of MDS assessments for all residents.
• Participate as an active member of the interdisciplinary team and collaborate with the DON.
• Support audits and care conferences to promote quality resident care.
