Job summary

Centene Management Company LLC is seeking a Senior Care Manager RN in Texas to assess, plan, and implement complex care management activities to enable high-quality, cost-effective healthcare outcomes. You will develop personalized care plans, coordinate with providers, and educate members and families about services and benefits.

The role requires 4–6 years of RN clinical nursing or care coordination experience; BSN preferred.

Qualifications

• RN with state licensure and 4–6 years in clinical nursing or care coordination.
• BSN degree preferred.
• Experience with Medicaid, STAR, CHIP a plus.
• Must reside in Texas due to occasional in-person trainings or meet-ups.

Responsibilities

• Develops and updates personalized care plans and collaborates with providers to identify needed resources.
• Coordinates between member/family and care team to ensure person-centered care.
• Monitors progress and revises care plans to meet goals.
• Identify problems/barriers and apply appropriate interventions for escalated cases.
• Reviews member data to identify trends and improve quality as per state/federal regs.
• Performs outreach via telephonic, digital, home visits, or other sites as needed.
• Collaborates with healthcare providers to modify care plans when needed.
• Maintains all member information and ensures regulatory compliance.
• Educates members and families on disease processes, care options, and benefits.
• Partners with leadership to improve care delivery cost-effectively.
• May precept new hires and assist onboarding.
