Qualifications

• Graduate of an Accredited School of Nursing and 4–6 years of clinical nursing, direct RN case management, and/or care coordination experience.
• BSN degree is preferred.

Responsibilities

• Develops and implements personalized care plans for members and educates families on services and benefits.
• Coordinate between member/family and care providers to deliver person-centered care.
• Monitor and revise care plans to meet member goals and needs.
• Collaborate with providers to identify resources and ensure quality and compliant care.
• Document and maintain member information and care management activities.
