BLEHEALTH is seeking a Lead Care Manager to coordinate Enhanced Care Management services for chronically ill members in the Metro Los Angeles area. You will work with members, families, providers, and community resources to implement individualized care plans and ensure timely access to care.

The role emphasizes field-based coordination, strong communication, and adherence to Medi‑Cal MCP guidelines while maintaining HIPAA compliance and working within the 20-mile service radius.

Competenze

• California driver’s license required.
• Reliable personal vehicle with ability to drive within 20-mile radius.
• TB test and CPR certification required.
• Live Scan fingerprint/background check.
• Ability to meet daily productivity expectations.

Mansioni

• Coordinate care across clinics, hospitals, and community agencies to ensure seamless service delivery.
• Oversee ECM service delivery and coordinate individualized care plans.
• Assist members with access to medical, behavioral health, and specialty care; arrange transportation.
• Attend discharge planning and follow-up with hospital staff.
• Document in EHR and maintain HIPAA compliance.
