Qualifications

• Graduate of an accredited school of nursing or Masters Degree in Social Work required.
• Two or more years clinical experience with one year in the acute care setting preferred.
• RN or LMSW in the state of employment is required for new hires.

Responsibilities

• Coordinate patient care progression throughout the continuum of care.
• Assess patient/family needs and develop individualized care plans.
• Act as patient advocate and coordinate resources with payors.
