Job summary

Qualifications

• Graduate of an accredited nursing program or MSW; required, with 5 years' related experience if education is limited.
• Two+ years of clinical experience; acute care preferred.

Responsibilities

• Coordinate case management and discharge planning across hospital departments.
• Advocate for patients with payors, providers, and community resources.
• Identify barriers to throughput and ensure timely discharge; collaborate with multidisciplinary teams.
