Job summary

Qualifications

• Graduate of an accredited nursing program or MSW required.
• Two+ years clinical experience; acute care preferred.

Responsibilities

• Coordinate case management into patient care and discharge planning.
• Monitor length of stay and advocate for patient needs.
• Collaborate with physicians, nurses, and staff to remove barriers to care.
• Initiate discharge planning at admission and coordinate post-acute referrals.
• Document and communicate care plans to patient/family and care team.
