Qualifications

• BSN degree required or demonstrated success in CHRISTUS RN Care Manager I position.
• Strong discharge planning and utilization management knowledge.
• Experience in case management and clinical practice.

Responsibilities

• Meets expectations of leadership competencies and coordinates care progression.
• Implements and monitors the care plan across the continuum.
• Identifies barriers to diagnostic or treatment progress and escalates.
• Promotes timely discharge and collaborates with care team.
• Communicates plan of care to patient/family and monitors resource use.
• Provides patient advocacy and transitions to next level of care.
