Responsibilities

• Coordinate discharge planning and care progression through hospital stay.
• Collaborate with physicians, nursing and ancillary staff to remove barriers to timely discharge.
• Advocate for patients and families, addressing social, emotional, financial needs.
• Identify barriers to care and coordinate resources with payors, agencies and vendors.
• Document and communicate the plan of care to the patient, family, and care team.
