Job summary

Neighborhood-Healthcare is seeking an ECM Lead in Escondido, CA to guide program development and oversee daily care management operations for the ECM program. You will mentor staff, coordinate with leadership and providers, and ensure compliant, patient-centered care.

You will develop linked care plans across medical, behavioral health, and social services, while coordinating with community partners to improve outcomes for high-utilizer populations.

Qualifications

• High school/GED required; two years community outreach or behavioral health experience with high-risk populations.
• Experience in patient-centered medical home or integrated primary care settings preferred.
• Lead experience preferred; LEAP familiarity is a plus.
• Valid CA driver's license and auto insurance required.

Responsibilities

• Train and mentor staff; respond to daily questions to keep ECM operations productive.
• Collaborate with management to ensure ECM operations run smoothly and meet goals.
• Develop care plans linking medical, psychiatric, social, and educational services as needed.
• Coordinate with community organizations to deliver team-based, patient-centered care.
• Support daily operations, staffing, scheduling, and partner liaison activities.
• Lead meetings/trainings; drive quality assurance, improvement, and compliance.
• Assist in hiring decisions and staff development; oversee case file audits.
• Conduct outreach, enrollment, and monthly risk assessments with care plans.
• Ensure accurate intake steps and proper care management within ECM guidelines.
