Qualifications

• A real passion for aging adults and quality of care.
• Genuine curiosity and ability to ask the right questions.
• Team-oriented problem solving with schedulers, RN, care pros.
• Comfort discussing hours and costs.
• Highly motivated and self-directed.
• Valid driver's license and clean driving record.

Responsibilities

• Be the first contact for families and assess needs.
• Build care plan, agree hours and cost, start services.
• Stay with client through the first month.
• Collaborate with schedulers, RN, and care team.
• Maintain ongoing communication with families.
