Job summary

Ardenwell Home Care in Houston, TX seeks an Intake Coordinator to conduct in-home admissions assessments, meet with clients and families, and develop personalized service plans based on needs and authorized services.

The role includes reviewing intake paperwork, coordinating with clients, families and leadership, ensuring compliant, smooth starts of care, and representing the agency at community events and outreach opportunities.

Qualifications

• Must be at least 18 years old.
• Authorized to work in the United States.
• Able to pass a criminal background check in accordance with Texas HHSC requirements.
• Able to complete pre-employment screening required by Ardenwell Home Care.
• Valid driver’s license, reliable transportation, and proof of current automobile insurance.
• Ability to travel throughout Ardenwell Home Care's service area for accepted assignments.

Responsibilities

• Complete in-home intake and admissions assessments for new clients when assignments are offered and accepted.
• Meet with clients and families to assess care needs, routines, preferences, goals, and requested services.
• Develop individualized service plans based on the client's assessed needs, preferences, and authorized services.
• Review required intake paperwork, client rights and responsibilities, agency policies, service expectations, and other admission information with clients or their authorized representatives.
• Communicate relevant client needs, concerns, preferences, and other information to agency leadership to support a smooth start of care and appropriate caregiver matching.
• Provide clients and families with a professional, compassionate, and welcoming experience while representing Ardenwell Home Care.
• Accept optional caregiver shifts when offered and based on availability, providing direct non-medical care in accordance with the client's service plan and agency policies.
