Mountain View PACE, part of the One Senior Care family, is expanding across Kentucky, seeking a compassionate Participant Enrollment Specialist. You will guide seniors and families through enrollment, coordinate documentation, and liaise with the interdisciplinary team to ensure a smooth process.

You’ll work with participants, families, and community partners in a supportive, mission-driven environment focused on confidentiality and high-quality care.

Formación

• Experience working with frail or elderly populations is a plus but not required.
• Knowledge of Medicare and Medicaid regulations is helpful.
• Experience with community outreach, enrollment, sales, or marketing is a plus.

Responsabilidades

• Meet with potential participants and families to explain the program and assess needs.
• Complete in-home and community-based assessments for prospective Participants.
• Coordinate required evaluations, documentation, and intake steps.
• Attend intake appointments and support Participants throughout the enrollment process.
• Complete intake and enrollment packets accurately and on time.
• Serve as the main point of contact for referrals and guide them through each stage of enrollment.
• Present referrals to the Interdisciplinary Team for review and ongoing collaboration.
• Maintain accurate records and communication updates in the CRM and EMR.
• Work closely with the Medicaid Enrollment Specialist to support a smooth transition to Medicaid processing and verification.
• Participate in team meetings, training, and quality improvement activities.
• Track weekly and monthly goals, enrollment progress, and key performance metrics.
• Help identify barriers to enrollment and work with the team to create solutions.
• Support additional projects and responsibilities as assigned.
