Job summary

Mountain View PACE, part of the One Senior Care family, is expanding across Kentucky. As a Nurse Practitioner, you’ll assess, manage, and coordinate the healthcare of chronically ill geriatric participants.

You’ll work closely with physicians, medical leadership, and clinical operations to ensure safe, accurate, and high-quality care while supporting an interdisciplinary team. This position is ideal for someone who is analytical, meticulous, and committed to delivering precise clinical results.

Qualifications

• Must be licensed as a Certified Nurse Practitioner in the state of practice.
• One year of experience working with the frail and elderly population or willingness to participate in extensive training.

Responsibilities

• Practice within defined scope and regulatory guidelines, including prescriptive authority.
• Perform health histories, physical exams, patient education, and follow-up on diagnostic results.
• Identify urgent or emergent medical conditions and initiate appropriate interventions.
• Collaborate with physicians and interdisciplinary team members to support optimal participant outcomes.
• Support clinical operations, including documentation, scheduling, and charting.
• Participate in quality improvement initiatives and care planning activities.
• Serve as a clinical resource for staff, participants, and families.
• Contribute to 24-hour care coordination.
• Attend meetings and complete other duties as assigned.
