Job summary

SCAN Health Plan is seeking a Patient Care Coordinator to support clinicians and coordinate care for members, helping them stay healthy and independent.

You will work with SCAN clinicians to coordinate care, arrange services and transportation, obtain records, manage calls, and ensure timely, high-quality service. This role offers remote/field work around Riverside, CA with a strong focus on patient-centered care.

Qualifications

• High School Diploma or equivalent experience.
• Associate's degree or equivalent experience preferred.
• 1+ years of related experience required.
• Bilingual Spanish proficiency is preferred but not required.
• Technical and problem-solving abilities; strong communication.
• Organizational and multitasking skills.

Responsibilities

• Coordinate essential patient care by processing orders and arranging services.
• Coordinate patient appointments and transportation with insurance and documentation.
• Obtain patient care records from hospitals, doctors, labs and vendors.
• Manage calls, mail, faxes and emails; prepare forms and refill prescriptions.
• Maintain inventory of supplies for clinical staff; stock and order as needed.
• Conduct home visits to complete assessments and deliver supplies.
• Communicate professionally with members, caregivers, clinicians, vendors.
• Seek improvements from patient feedback and propose solutions.
