Job summary

SCAN Health Plan seeks a Care Manager to coordinate care for members in collaboration with the Integrated Care Team. You will ensure timely, high-quality care, authorize services, and guide patients through appropriate facilities with end-to-end coordination.

This full-time, remote role requires an LVN license in California and 3–5 years of related experience. You will apply clinical judgment, participate in high-risk care management, and support care coordinators while advancing SCAN’s mission.

Qualifications

• Associate's Degree or equivalent experience.
• 3–5 years of related experience.
• CA LVN license.
• Performs work under direct supervision.
• Provides leadership, coaching, and/or mentoring to partner care coordinators.
• Demonstrates problem-solving and organizational skills.

Responsibilities

• Serve as point person to speak with patients, family members, and providers about care, transfers and authorizations.
• Ensure inpatient/outpatient care is rendered at the right time and location with end-to-end coordination.
• Coordinate discharge needs from inpatient settings and skilled nursing facilities.
• Coordinate timely delivery of last cover day letters to SNFs.
• Apply evidenced-based criteria and clinical judgment to develop treatment plans.
• Review tasks from Integrated Care Nurse Practitioners and coordinate services (home health, DME, wound care, etc.).
• Review appeals and create explanations for non-coverage and reinstatement letters.
• Refer cases to Medical Directors for secondary review when criteria aren’t met.
• Maintain high-risk patient lists and conduct regular outreach calls.
• Develop working relationships and provide clinical guidance to Care Coordinators.
• Support SCAN’s mission and engage with external stakeholders and vendors.
