Job summary

SCAN Group is seeking a Care Coordinator to support SCAN members in aging in place. You will assess, plan, coordinate, and monitor holistic needs, handling referrals and services, while helping members navigate the healthcare system.

Responsibilities include triage, collaboration with providers and county offices, and ensuring timely documentation across systems. Strong communication and confidentiality are essential.

Qualifications

• High School Diploma required; Some college preferred.
• 0-3 years working in healthcare/customer service.
• Spanish strongly preferred: Conversational, Reading, Writing.
• Microsoft Office skills (Word, Excel, PowerPoint, Outlook, Teams) are required.

Responsibilities

• Perform assessments and referrals using motivational interviewing to understand member goals.
• Act as triage and contact point for benefits/program administration and access issues.
• Coordinate care utilizing member benefits and community resources.
• Document assessments, care plans, and case notes in CMS and internal systems on time.
